• Who committed the alleged wrongdoing? Was anyone else involved? Provide full names and titles if possible
• When and where did it happen? Indicate dates and times, if available
• How did the individual or firm commit the alleged wrongdoing and why do you believe the activity was improper?
• Is there any supporting evidence? If possible, provide information on documentation available to corroborate the allegations, as well as names of witnesses to contact
